International students that have been admitted to Ursuline College are required to submit this form to receive a Certificate of Eligibility (Form I-20) in order to apply for an F-1 Student Visa. Students must submit this document in addition to a copy of their biographical passport page and their proof of finances. You can upload these documents in the space provided below.
